Corneal Confocal Microscopy: a Surrogate Endpoint for Neurodegeneration in Clinical Trials

Immune-mediated neuropathies, such as Chronic Inflammatory Demyelinating Polyneuropathy (CIDP), are treatable conditions of the peripheral nervous system. There is an unmet need for better diagnostic tools to further delineate clinical subsets of heterogeneous group of immune mediated neuropathies and surrogates to detect ongoing inflammation. This talk will present data, evaluating the potential of corneal confocal microscopy (CCM) as a diagnostic surrogate. Current data suggests, that corneal nerve fiber loss may reflect severity of neuropathy. Quantification of distinct cells around the corneal nerve fiber plexus may help in stratifying CIDP subtypes, clinical course, and disease activity. Particularly, dendritic cells in proximity to corneal fibers seem to be a potential surrogate to differentiate inflammatory from non-inflammatory neuropathies. However, further longitudinal studies are warranted to elucidate the clinical potential of CCM for patients with Immune-mediated neuropathies.
